Care Plans and the Informed Patient Data

For protocols defined in Care Plans to be effective they must be an integral part of the system. At the same time they need to allow carers navigational freedom, treatment protocol exceptions, and only offer advice when deviations from department agreed “best practice” occur. The screenshotsbelow shows how one data item is related to different users, work-flow, the conceptual Care Plan and informing the patient, in an ophthalmic setting. This is also a good example of an aspect of product development requiring input from different participants to achieve an optimal solution.

""1 At the end of an outpatient consultation, the clinician sets a follow up schedule and orders interim investigations, validated against the applicable protocol.
""2 Each item follows it’s own path, for example transferring the visit interval to the booking system. Selecting an item opens an “Order Form” with the available options.
""3 The completed Order Form is added to a “Requests” interface used by other team members, here an imaging technician. This information is also added to the printable Care Plan in an appropriate form.
4 The Order Form is opened from the Requests interface and the investigation performed. The observation, in this example an optic nerve head image, is linked to the patient and saved in one or more databases.

""5 At the next outpatient consultation, the observations to be made, investigations requested in the interim, and any overdue care activities are listed.
""6 Depending on individual preference, the user selects a starting point. Clicking the ONH link opens the data entry section and image viewer. Annotations can be added to the image.
""7 The user performs the remaining activities required by their role. In this case it will be a clinician who will Order further investigations and complete the follow up schedule for the next visit.
""8 The printable Care Plan is for the patient and summarises progress and what will happen as a result of the consultation.
